Health Insurance Dates

Hello,
I had to get temporary residence extension (article 14) for my child due to delay in passport renewal.
Now that passport is renewed, I want to renew normal residence (article 22).
My query is regarding the dates for health insurance online. What date should I put for starting and ending of health insurance? Should the date be as per the original dates of the previous iqama (which expired in April) or it should be starting from today and ending 1 year from today?

Please advice.
