you have to go to above link. Carry Birth certificate, Civil ID Father, Mother, Kid, Vaccination card and 2 photographs. No typing required. At counter they will give application fill. After filling you have to go inside and had over to Doctor. Doctor will check the kid and issue Health card.

I have done the below process recently. My child is Kuwait born. We are from Farwania Governorate.
1. Go to MOH site and take appointment for School admission checkup. 2. You will get appointment for next day any time. 10 mins gap for each appointment. 3. You will see only one clinic name, which you need to select. However, you need to go to Ishbilya Clinic. 4. Ishbilya clinic you can get in Google Map. 5. Documents to carry- All original certificates of Civil ID (parents and Kid) Birth Certificate of Kid and also Passports. 6. Photocopies to carry: All the above and Birth certificate copy with Vaccination page and 2 blue background PP size copy.

Dear All, Below is the process at Al Bashaer Kindergarten, St 1, Bayan. I took an appointment for the 27th of Jan @ 8:30 AM.
We went to the reception, where a man took all the documents copies and prepared the file. He gave the file and asked us to go to Hall Number 1, Room 2, where the nurse verified all the documents with originals.
My son is India-born and looking at the copies she asked if he was given an injection (Meningitis) while doing the medicals procedure in Kuwait. I had the copy of the medicals paper and was sure they didn''t give the injection and also the stamp given on the paper from Sabah only mentioned BCG before and polio drops. The lady then asked us to visit the Local Clinic (For people staying in Salmiya Block 10, It''s Sultan Center Clinic) and ask for a Vaccination list card and tell them it''s for opening a Health file for school.
We went to the clinic and informed the purpose to the receptionist, she told me to take the child to the pediatric section. The nurses there again confirmed if he was given an injection at Sabah Health Center and looked at the vaccination copy. Looking at it, they confirmed the vaccine was not given and vaccinated my son for Meningitis and updated the same on the Indian Vaccination card.
After the process, we left for the Bayan office again. I showed the updated file and handed over the new copy to the nurse. Verifying the same, she took us to the Doctor who was in a room adjacent to her, she crossed checked the details and gave her stamp and told us to go to the reception again. At the reception, the man again stamped the papers and asked to submit them to the school. The process was thus completed for us and was submitted to the school on the same day.
Docs required are the same as per the list given by the school: 1) School Request Letter 2) Copy of latest Civil ID of Father & child 3) Copy of birth certificate & Passport 4) Copy of Vaccination card if born outside Kuwait 5) 2 blue background PP size Photographs (Only 1 is to be used).
Please Note: Originals are to be carried for verification. The appointment is automated to the office as per the child''s address on Civil ID & Childs presence is mandatory at the Office.
My recommendations: Visit the local clinic to confirm if any vaccination is pending for the child before visiting the office for the school''s health file.
